HealthTrack SG - Programme Participation Agreement (PPA)
Programme Overview
Here are some key facts about the HealthTrack SG programme which is a digital health and wellness
programme (referred to as the “HealthTrack SG Programme” or “Programme”) administered by Health
Promotion Board (“HPB”) in collaboration with Google, ConnectedLife Pte Ltd (“ConnectedLife”) and
Fullerton Health Pte Ltd (“Fullerton Health”) to help improve the management of chronic conditions
through continuous monitoring of health and lifestyle habits via HealthTrack SG mobile application (the
“App”). By tapping ‘Agree,’ you acknowledge that you understand and agree to participate in the
HealthTrack SG Programme. ‘Programme Partners’ refers collectively to HPB, ConnectedLife, Fullerton Health, and Google.
- Your participation is entirely voluntary. You can choose to withdraw at any time.
- The Programme lasts for twelve (12) months, commencing from the date your first Wellness Plan is issued during your doctor’s consultation.
As a participant, you are consenting to allow HPB to collect, use, and disclose your Programme Data (as
defined hereafter) as outlined in this agreement.
You may be prompted to provide consent to a new Programme Participation Agreement if there are changes to
the Programme and you may be withdrawn from the Programme to the extent such PPA is not agreed upon.
About the Programme
The Programme is a twelve (12)-month programme offered in partnership between HPB, Google, ConnectedLife,
and Fullerton Health. It is a digital health and wellness programme that empowers patients and their
doctors to examine how lifestyle changes can help patients manage their chronic conditions. Through the Programme, participants will be able to track their health progress, gain a holistic view of their
health, and receive personalised guidance through the use of a wearable device and the App.
To participate in the Programme, you must meet the eligibility criteria and agree to this Programme
Participation Agreement (PPA).
Eligibility Criteria
You are eligible to participate in the Programme if you:
- Are a Singapore Citizen or Permanent Resident between the age of 25 to 75 years old with a valid
Singpass account;
- Have been informed by a healthcare provider or are otherwise aware that you have any of the following conditions:
- High Blood Pressure (Hypertension) or High-normal Blood Pressure
- Type 2 Diabetes Mellitus or Pre-Diabetes
- Lipid Disorders/High Cholesterol
- Are comfortable with spoken and written English;
- Are comfortable using a supported health tracker during daily activities and sleep;
- Are using a smartphone with iOS 15 or Android 10 and above, compatible with the HealthTrack SG App,
HPB Healthy 365; and a compatible wearable device;
- Agree to HealthTrack SG Terms of Use.
HPB reserves the right to select participants based on Programme requirements and to discontinue participation at its sole discretion.
Programme Participation Requirements
The Programme lasts for twelve (12) months, commencing from the date of successful onboarding. Onboarding
is considered successful once all of the following steps have been fulfilled:
- Download the HealthTrack SG App;
- Complete Onboarding Survey within the HealthTrack SG App;
- Link a compatible wearable device to the HealthTrack SG App;
- Schedule and attend a medical consultation at any participating Fullerton Health clinic to undergo
the required blood tests and receive your first personalised Wellness Plan from the assigned
clinical doctor;
The Programme requires active participation, which includes:
- Wearing a supported wearable device* during daily activities and sleep;
- Syncing your wearable device regularly with the HealthTrack SG App;
- Log in to the HealthTrack SG App regularly to monitor your Wellness Score, read Health Messages and access personalised behavioural nudges;
- Participating in consultations with your assigned clinic’s healthcare professional, who will have
access to your dashboard data;
- Completing the biweekly Report Health Status in the HealthTrack SG App;

Please note that you may be withdrawn from the HealthTrack SG programme if you (i) do not wear and sync your wearables with the HealthTrack SG App and/or (ii) complete the biweekly Report Health Status in the HealthTrack SG App for more than 90 days. Following your withdrawal from the Programme, future visits with your healthcare provider at Fullerton Health will not be part of the Programme and you will no longer be eligible for any rewards under the Programme.
Programme data collected
By agreeing to participate in the Programme, you acknowledge that the information listed below
(‘Programme Data’) will be collected under the Programme.
Programme Data may include information such as the following data types:
- Contact Information: This may include information you provide as part of signing up
for the Programme, such as your name, NRIC, age, sex, ethnicity, email address, contact number.
- Singpass Data: This may include information such as Singpass ID.
- Lifestyle Data: This may include information provided via the Onboarding Survey and
the biweekly Report Health Status survey such as smoking status, alcohol, diet, stress levels.
- Your Wearable Device app Data: This may include information in the list below. You will be able to choose which fields to grant the app the ability to read and write during the setup process. Note that if you allow the app to read the fields listed below, you are also agreeing to share historical data for the past 90 days, if that data is available. Information requested to be shared includes:
- Activity data like standing, walking, exercise time, distance
- Heart measurements like heart rate
- Sleep analysis
Sharing your Wearable Device app’s data is required to participate in the Programme.
- Clinical Data: This may include information you provide via the Onboarding Survey when you sign up for the Programme, and biweekly Report Health Status survey, such as blood pressure, resting heart rate, heart rate variability, fasting blood glucose, HbA1c, cholesterol, triglycerides, weight, BMI, waist circumference. For the avoidance of doubt, clinical data may also include biological and physical information recorded by your doctor or clinic assistants during your medical consultations.
- Self-Reported Health Information: This may include information you provide via the Onboarding Survey when setting up a HealthTrack SG account for the App, and information you provide through the biweekly Report Health Status survey received through the App
- App Engagement and Usage Data: This may include information such as when and how frequently the app is used, syncing of wearable data, participation and responses to the biweekly Report Health Status survey, information about notifications received such as content, frequency, responses to notifications, user profile, device model, software, the App version, and any other relevant App-related logs. This Programme Data will be stored in Singapore.
What purposes may my Programme Data be used for?
Your Programme Data will never be sold. The Programme Data that you share may be used to support, assess,
and evaluate the Programme, including, for the following purposes:
- To provide you with services and functions of the Programme, including setting up your account and
managing your use and access, informing you about service updates, and managing and providing
guidance on lifestyle change,
- To identify and deliver Programme-related messages that may be of interest to you,
- To identify correlations between Programme Data collected and messages delivered as part of the
Programme,
- To inform you of Programme updates and announcements, which you can manage through notifications or
email preferences in the App,
- To inform you of changes to the Programme and deliver other important messages,
- To develop and refine the Programme,
- To understand overall Programme effectiveness and/or its impact on the healthcare system in
Singapore,
- To assist you with enquiries and obtaining your feedback,
- To devise, organise and implement, either independently or in collaboration with other organisations, programmes and activities that promote good health, healthy lifestyles and the prevention or early detection of diseases among the people of Singapore. This includes conducting health education initiatives, supporting research and monitoring of health and nutrition trends, encouraging healthier food choices, setting and recommending nutritional standards and dietary guidelines, providing healthcare services such as medical, dental, screening and immunisation programmes for schoolchildren and other groups, offering consultancy on health education, wellness and disease prevention, and representing Singapore internationally on matters related to health promotion and disease prevention.
If you are participating in the Programme through your employer, please note that HPB may prepare and share with your employer an aggregated corporate health report based on the Programme Data of employees who are participants. This report will only contain aggregate-level information such as average HbA1c, blood pressure, BMI, LDL levels, as well as overall programme participation and completion rates. No individual-level or personally identifiable information will be shared with your employer.
If you decide to participate in the Programme, HPB, the Programme Partners and their respective subcontractors may use your Contact Information, such as your name and email address, to contact you to:
- Obtain feedback on your experience with the Programme, wearable device, the App, and related
matters, and/or
- Inform you about research studies conducted by HPB. Joining these studies is entirely voluntary and
will not affect your Programme participation. You can opt out of receiving future research study
communications at any time.
Who will have access to my Programme Data?
HPB and the Programme Partners will have access to the Programme Data, which may be used for the purposes set out above.
HPB may also share your Programme Data with the Government of the Republic of Singapore and all organisations for all matters connected with the promotion of good health and healthy lifestyles among the people of Singapore, including advising the Government on health-related policies and initiatives, and developing and carrying out programmes and activities that encourage good health, healthy living, and the prevention or early detection of diseases. This includes health education efforts, nutrition and dietary guidance, and collaborations with other organisations to support these aims. It also covers monitoring and researching the health and nutritional status of Singapore residents, encouraging healthier food choices, setting nutritional standards and dietary guidelines, and recommending ways to provide clear nutritional information. In addition, it includes providing healthcare services such as medical, dental, screening and immunisation services to schoolchildren and other groups, offering consultancy services to Government agencies, the healthcare industry and the private sector on health education, health promotion and disease prevention, and representing Singapore internationally on matters related to health, disease prevention and health education.
Further, HPB may use or disclose your Programme Data as allowed at law. For instance, HPB may use or
disclose your Programme Data to third parties in the national interest.
What other terms must I know related to my participation in the Programme?
When you participate in the Programme, you are required to comply with its Terms of Use, which are available at https://hpb.gov.sg/healthy-living/healthtracksg/terms. Please also be aware that your personal data will be processed in accordance with this PPA and the HPB’s Privacy Statement, accessible at https://hpb.gov.sg/privacy-statement.
How long does my PPA last?
- Your PPA remains valid from the moment you agree to participate until the Programme ends. It covers any Programme Data collected during that period.
- We may ask you to sign a new PPA if the Programme undergoes significant changes, such as its structure, collected data categories, or how your data is used.
Are There Any Account Restrictions or Termination Conditions?
The terms in this PPA allow HPB to restrict your account access if you do not comply with Programme
requirements (listed in section above) or if your account becomes inactive.
- Remaining inactive: If you stay inactive more than 90 days, you may be withdrawn from
the Programme. Please note that HPB reserves the right to withdraw participants at its discretion and without prior notice. Any collected Programme Data will still be stored and used as outlined in the PPA.
- During inactivity: Some Programme Data may still be collected, stored, and used under
the current PPA.
- If intentional fraud or any suspected attempt to manipulate the mechanics of the Programme or Programme Data is detected, such as inaccurate BP or cholesterol levels, your participation in the Programme may be terminated. Examples include, but are not limited to, providing false information about your health status, intentionally misrepresenting your activity levels, or falsifying data related to your participation in the Programme.
What if I no longer want to participate?
If you do not wish to continue with the Programme, you may withdraw by:
Deleting your HealthTrack SG account
- Go to My Profile, then select Privacy Policy in your HealthTrack SG App. Click Delete Account to withdraw from the HealthTrack SG programme.
- Please note that simply uninstalling the HealthTrack SG App will not lead to deletion of your account or
associated Programme Data.
What happens when you withdraw?
- Programme access: Your access to the Programme on the App will be terminated.
- Programme Data collection: HPB will stop collecting your data after withdrawal
within 24 hours.
- Programme Data already collected: HPB may keep and use data collected before your
withdrawal as outlined in this PPA.
Following your withdrawal from the Programme, future visits with your healthcare provider at Fullerton Health will not be part of the programme and you will no longer be eligible for rewards.
Please also note that by withdrawing from the Programme, HPB will stop accessing any new Programme Data
as outlined above.

ONBOARDING SURVEY
- What is the Onboarding Survey?
The Onboarding Survey is a required step in the onboarding process within the App that collects key demographic, medical and lifestyle information to compute your baseline Wellness Score.
REPORT HEALTH STATUS
- What is Report Health Status?
The Report Health Status is a self-reporting feature in the HealthTrack SG app that collects lifestyle (e.g. diet, smoking, alcohol and stress levels) and clinical (e.g. weight, waist circumference, blood tests [if any]) data to update your Wellness Score. It is accessible under ‘My Workspace’.
WELLNESS PLAN
- What is the Wellness Plan?
The Wellness Plan is a personalised care plan co-developed by you and your doctor. Based on your profile, preferences and health data, your doctor will recommend up to three lifestyle actions for you to try.
WELLNESS SCORE
- What is the Wellness Score?
A Wellness Score is calculated through a proprietary algorithm determined by lifestyle parameters of your wearable device such as step count, physical activity, sleep as well as clinical data like your heart rate and blood pressure to give you an indication of your overall wellness and areas for improvement.
